Deadline for 2025 submission: March 17, 2025
Please answer all questions listed on the form below. Consider creating your answers in a word document and then copying/pasting into the online nomination form to avoid lost work. The background information will be included on the ballot so that voters can learn more about the qualifications of each of the candidates. The information provided within each section of the form may vary among candidates; however, the form must be kept to a one-page maximum.

Qualifications:
A candidate for an elected leadership position must be an Academy member in good standing, this includes a member complies with the Academy’s ethical principles and has maintained an active membership status and a grace period of up to 3 months maximum for any lapses will be assessed.
