New Resources Posted from the Knowledge Translation Task Force for the Core Measures CPG

Pocket Cards: These are referenced handouts with data to assist with the interpretation of core measure scores. These include normative values, cut-offs, and meaningful change data which have been established across various neurologic health conditions. The information may be used, in conjunction with a full PT exam, to draw conclusions, set goals, and educate your patient. The pocket cards print to the size of a half-sheet of paper. Patient Report Card: The interactive report card can assist clinicians (electronically or on paper) with documenting a patient's change on the core measures over time. The Report Card is patient-friendly and includes averages for community-dwelling adults and/or healthy adults and potential indications for fall risk based on current referent values.
